An employer's representative was not able to locate a natural gas service line (1.25-inch diameter) by consulting the BC 1 Call documentation. When they were also unable to locate it by hand, they tried to remove a top layer of ground using powered equipment. The equipment contacted the line, resulting in a release of natural gas.

A forklift operator was loading wooden bins filled with fruit into a cold storage room when they noticed the support point of a previously loaded bin fail, causing a stack of bins to topple onto a section of the refrigeration system. A valve (3/8-inch gauge) broke off the oil pot vapour evaporator, causing an uncontrolled release of ammonia. The workplace was evacuated, and the fire department attended and assisted with the isolation and shutoff of that section of the refrigeration system.

A worker was delivering stone slabs at a stone shop. The worker got out of their truck and stepped out onto the flatbed to unload the slabs. The worker pulled two quartz slabs toward themselves to make room so they could put the clamps of a lifting attachment onto the slabs. The slabs tipped toward the worker, and the worker fell from the truck to the ground. The slabs fell and landed on the worker, causing fatal injuries.

A reaction ferry with two workers aboard experienced a failure of the main cable (counterweight cable) that guides the ferry. The cable broke during operation mid-way across the river, stranding the vessel in the middle of the river. There were no passenger vehicles aboard the ferry at the time of the incident, and the two workers were able to use the attached rescue boat to evacuate the vessel and get to shore.

The machine was shut down but left in service mode so that the ram plate could be manually moved up and down using a remote control. One of the workers was operating the remote, and the other (a young worker) was at the opposite side of the press, removing bolts. When the first worker activated the remote to raise the drum, the second worker's hand was caught between the press and the drum.
